Obtain Application
An application form is required for program registration. The first part of the application is to be filled out by individual needing services. The final part is to be filled out by the applicant's physician.
- Fill out and print the online application form (446 K)
., or - Pick up the required application from the League of Human Dignity (1701 'P' Street).
Submit Completed Application
The completed application should to be returned to League office. There are two methods of submitting the application:
- Bring the application into the League office (1701 'P' Street). Staff will take the picture and issue the ID card immediately.
- Complete applications and a small photo can be mailed to the League office. Staff will mail to the individual the ID card and the Handi-Van Operating Guidelines brochure.
