Information & Assistance
Information and Assistance /First Services is the starting point for obtaining information, determining alternatives, and arranging services for individuals age 60+ and their families in an eight county area. For other human service professionals, First Service is the contact point for all referrals for any of Life's programs. Our goal is to provide other professionals a quick, easy means to request services, consult, and coordinate. First Service is open from 9:00 a.m. to 4:00 p.m. on Monday, Tuesday, Thursday, and Friday for walk-in consultations or phone calls. To accommodate regular weekly training, the service is only available between 12:30 p.m. to 4:00 p.m. on Wednesdays.
Counseling & Social Work
Counseling and Social Work furnishes intensive, temporary, professional assistance to address difficult changes, surmount personal obstacles, and resolve serious multiple-problem situations.
Care Management
Care Management helps vulnerable elderly people remain in their homes. Care planning services begin with a comprehensive assessment of the elderly person's needs, abilities, circumstances, and support. From the assessment, a detailed plan for home care is developed. With the elderly person's permission and participation, the Aging Partners Personal & Family Services counselor arranges and coordinates the delivery of various in-home services. The elderly person's needs and care plan are reviewed regularly. Adjustments are made, as necessary, to maintain his or her safety and quality of life at home on a long-term basis.
