Your First Appointment
- You will be given an overview of services available from YESS
- Meet with staff to discuss basic eligibility requirements
- Complete an application and provide documentation to prove your eligibility
What should I bring to my first meeting?
- Picture ID
- driver’s license,
- state ID,
- military ID, OR
- school ID and birth certificate.
- Social Security Card
- Proof of Low Income
- paystubs for the previous 6 months, OR,
- proof you are receiving public assistance:
- ADC/TANF
- SNAP (food stamps) in the last 6 months
- Supplemental Security Income
- Refugee Cash Assistance
- County General Assistance
- Proof of your current address, if not on your ID
- utility bill,
- rental agreement, OR
- other item mailed to you
- Proof of barrier to employment or education:
- Doctor’s verification of a disability or pregnancy
- Your child’s birth certificate
- High School transcripts showing drop-out status
- Court involvement
- probation, diversion or parole paperwork,
- copy of charges, OR
- copy of judgment
- Foster Child/State Ward
- order placing you under the care of the state or tribe
- letter from your Health and Human Services case worker
- other official paperwork showing State Ward status
