Absentee Ballot Request Form

Nebraska State law does not allow requests for an early/absentee ballot to be taken without an original signature.

Please note:
The early/absentee request form must be completed and in our office by Wednesday before the election at 4:00 pm. That's the deadline for us to mail out an early/absentee ballot. After that time people must personally come into the election office and vote early/absentee or pick up a ballot. The Ballot must be completed and returned by 8:00 p.m. on election day.

Please pick one of the two options below for the early/absentee ballot request form.

Option 1:
Please download the Lancaster County Nebraska Early/Absentee Ballot Request Form, print it, complete the form, sign it, and then mail or fax the form to the Election Office by the deadline.
