Guidelines (As of July 29, 2008) Application Attestation Form (Required as of Oct 1. 2009) Sample Checklist Physician Confidential Report Form Appeals Form		Food; Shelter (including deposit, rent and utilities); Assisted living (cannot be authorized without a written statement from a physician indicating the client is in need of the level of care provided by an assisted living facility); Medical care provided through the Primary Health Care Clinic or authorized by a Clinic Physician; Transportation; Non-food necessities (including household supplies and personal care items); Clothing; Cremation/Burial expenses; COBRA or other health insurance payments.
