WIC is the nation's premiere public health nutrition program which helps low to moderate income families eat well and stay healthy by providing: 

  • Nutritious foods 
  • Nutrition, health and community resource information 
  • Breastfeeding support 

to: 

  • Pregnant Women 
  • Breastfeeding Women 
  • Newly Delivered Mothers 
  • Infants 
  • Children - Up to age 5

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

(1) mail: U.S. Department of Agriculture Office of the Assistant Secretary for Civil Rights 1400 Independence Avenue, SW Washington, D.C. 20250-9410; or
(2) fax: (833) 256-1665; or (202) 690-7442; or
(3) email: program.intake@usda.gov.

Who is Eligible

  • Pregnant Women 
  • Breastfeeding Women - up to the infant's first birthday 
  • Newly Delivered Mothers - up to six months after the birth of the infant or the end of the pregnancy 
  • Infants 
  • Children - up to age 5 
  • Foster Infants and Children - birth to age 5 

Also... 

  • You must live in Nebraska 
  • Have a nutrition or health risk - WIC staff will complete an assessment 
  • Meet income guidelines
  1. Currently enrolled in Medicaid, TANF or SNAPOR 
  2. Have a before tax (gross) household income at or below the guidelines established by the USDA:

WIC Foods 

The WIC foods offer a variety of healthy choices. Your clinic will issue you "WIC checks" that can be used at participating stores. 

WIC Checks are accepted at stores that display this sign 

  • Fresh fruits and vegetables 
  • Whole grain bread, brown rice, and tortillas 
  • Beans or peanut butter 
  • Iron fortified cereals 
  • Milk, cheese and eggs 
  • Baby food fruits and vegetables 
  • Infant formula

Locations & How to Apply

Step 1: Find a WIC clinic

 


Locate a Nebraska WIC clinic by visiting the Nebraska WIC Program.

 

Step 2: Call to make an appointment 

Call 402-441-6200 to make an appointment. 

  • No long waits for appointments. Openings are available.  
  • Application appointments take 30-45 minutes. 
  • Our friendly WIC staff will answer all your questions. 

 

Step 3: Bring the following 

    1. You and your children who are applying for WIC 
    2. Identification 
      • Photo identification for parent/guardian 
      • Child's birth certificate or social security card 
      • Foster care papers for foster children 
    3. Proof of address 
      • Mail with your name and street address received within the past 30 days 
      • Current utility or cable bill 
    4. Proof of before tax (gross) income for everyone living in the home 
    • Pay stubs 

    You are encouraged to reschedule if you or your child is ill. 

    Breastfeeding, Community and Nutrition Resources

    Breastfeeding is good for mom and great for baby. Breast milk is the perfect food for your baby's first year of life and provides many health benefits such as: 

    • Fewer infections 
    • Less tooth decay 
    • Fewer allergy problems 
    • Fewer earaches 
    • Fewer weight problems 

    The staff of your WIC clinic is there for you, to answer any questions you might have. WIC staff is trained to help moms with breastfeeding questions and offer needed support. 

    Breastfeeding mothers also receive extra foods to nourish mom at this important time.
