Chronic Disease was identified through the Community Health Assessment process as one of four priorities for the 2022 Lincoln-Lancaster County Community Health Improvement Workplan. The Strategic Focus for this Priority is Diabetes, Heart Disease and Healthy Lifestyles. After further workgroup discussions, common objectives and action strategies narrowed the focus for prevention and management initiatives to Worksite Wellness and Affordable Youth Activities to reach a larger population. Access to Education and Resources became an integral part to include in the planning process. These workgroups meet regularly and set Objectives and Action Strategies each quarter.
