Chronic diseases, such as heart disease, cancer, and diabetes, are the leading causes of death and disability in the United States. In Nebraska, nearly 4 in 10 adults have 3 or more chronic diseases. Chronic disease makes learning and working more difficult, is expensive to treat, and puts a strain on our health care system. Although chronic diseases are among the most common and costly of all health problems, they are also among the most preventable.

Our Chronic Disease Prevention program works to prevent or delay the onset of chronic conditions that limit activities of daily living. Health education programs encourage healthy living, embrace health screenings for early detection of disease, and encourage appropriate self-management of existing diseases. Program activities include community education on topics such as:

  • Increasing physical activity and developing healthy eating habits
  • Diabetes Prevention or Control(PDF, 3MB)
  • Learning to manage chronic health problems
  • Information and referral for cancer screening (specifically breast, cervical, and colon)

Program activities are successful because of the collaboration with local health clinics, hospitals, businesses, schools, the Nebraska Department of Health and Human Services, non-profit organizations, community and cultural centers, and other City departments who work together to improve the health of the community.
