Chronic diseases, such as heart disease, cancer, and diabetes, are the leading causes of death and disability in the United States. In Nebraska, nearly 4 in 10 adults have 3 or more chronic diseases. They make learning, working, and enjoying life more difficult, are expensive to treat, and put a strain on our healthcare system. Chronic diseases are also among the most preventable health conditions. Small lifestyle changes, routine screenings, and community resources can help you stay healthier longer.

Our Chronic Disease Prevention program works to prevent or delay the onset of chronic conditions that limit activities of daily living. Health education programs encourage healthy living, prioritize health screenings to catch illnesses early, and encourage appropriate self-management of existing diseases. Program activities include community education on topics such as:

  • Increasing physical activity and developing healthy eating habits
  • Diabetes Prevention and Management Partnerships
  • Learning to manage chronic health problems
  • Information and referral for cancer screening (specifically breast, cervical, and colon)

Program activities are successful because of the collaboration with local health clinics, hospitals, businesses, schools, the Nebraska Department of Health and Human Services, non-profit organizations, community and cultural centers, and other City departments who work together to improve the health of the community.
