The Lincoln-Lancaster County Health Department wants to hear from you! If you are 18 or older and a resident of the county we want to know how you feel about living in Lincoln and Lancaster County. To help us, please complete a short, 10- minute survey. The survey is voluntary and you will not be identified in any way; also, no one will call. If you have previously completed this survey, please do not complete it again, but feel free to share the link with family and friends or inform them about the survey as we would also like their input.

The survey starts out with some health-related questions; however, its focus is on factors related to the quality of life in our community. Survey results along with other information we gather about aspects of the community will be used to identify the most pressing health issues that can be addressed through community action.
