Aging Partners has eight counties in our service area. Aging Partners works with each county to create local plans based on the population and resources in their communities.
Each county is jointly sponsored by County, State and Federal funds made available through their County Board of Commissioners or County Board of Supervisors, and Aging Partners.
Contributions for services and donations also provide support.

Congregate Meals
A noon meal is available for individuals age 60 and over for a suggested contribution. Call 402-367-6131 one day in advance to make a reservation. Meals are also available for those under age 60 for a fee.
There are extenuating circumstances that may qualify someone to receive a noon meal. Please call and visit with the Butler County program manager for details.
A salad bar is offered on Tuesdays and Thursdays for individuals age 60 and over for a suggested contribution and a fee for those under age 60. Call for details.
Home-Delivered Meals
These meals are available throughout Butler County. Each meal is one-third of an individual's recommended daily nutritional needs. The meals are delivered by volunteers dedicated to helping others. Modified diets are also available with doctor's orders. Call for more information or one day in advance to order a meal.
Mom's Meals
Home-delivered meals are offered for a suggested contribution. These meals are available to qualifying senior residents who reside in Butler County. Call Butler County Senior Services for more information.
Health and Fitness
Foot care clinics, blood pressure screenings and hearing aid services are offered at various times during the month. Special presentations and programs, many focusing on health education, are offered periodically throughout the year.
Tai Chi classes on Tuesdays and Thursdays.
Handi-Van
Public transportation for all ages is provided by the Butler County Handi-Van. Service is roundtrip from your home to David City, Omaha, Lincoln and Columbus. For complete details, cost and scheduling call 402-367-6131.

Senior Centers in Fillmore County
Senior Centers in Fillmore County are located in Exeter, Fairmont and Geneva. The senior centers are community meeting places where adults can come together for fellowship, group meals, learning and recreation. A noon meal is available to individuals age 60 and over for a suggested contribution. Meals are also available for individuals under age 60 for a fee.
There are extenuating circumstances that may qualify someone to receive a noon meal for a suggested donation. Please call and visit with the Fillmore County program manager for details.
Special services and activities at the senior centers include educational presentations, arts and humanities programs, community service projects, card parties, bean bag tournaments, blood pressure checks, pedicures, manicures and inter-generational activities.
Home-Delivered Meals
Meals are available throughout Fillmore County. Each meal is one-third of an individual's recommended daily nutritional needs. Meals are delivered by volunteers dedicated to helping others. Care Management Services
Individuals age 60 and over often find they need assistance when changes occur in their health or ability to function in the home. Care Management is one-way families and older adults can find the help they need. Services include:
- Information on programs, benefits and services.
- Help in locating, arranging and monitoring those services.
- Financial counseling, including whether an individual may qualify for publicly-funded programs, prescription assistance or community-based programs.

Senior Centers in Polk County
Polk County Senior Services invites all older adults to visit our senior centers for access to social, educational and recreational programs that help enhance their quality of life. Senior Centers in Polk County are located in Osceola, Polk and Shelby. A noon meal is available Monday through Friday at all senior centers for individuals 60 years of age or over for a suggested contribution. Meals are also available for individuals under age 60 for a fee. Please call a senior center one day in advance to make a reservation.
There are extenuating circumstances that may qualify someone to receive a noon meal for a suggested donation. Please call and visit with the Polk County Program Manager for details.
Special services and activities at the senior centers include educational presentations, health assessments, regular blood pressure checks, pedicure clinics, exercise classes, craft classes, pool tournaments, BINGO and much more.
Home-Delivered Meals
Meals are available throughout Polk County. Each meal is one-third of an individual's recommended daily nutritional needs. Home-delivered meals are available seven days a week in the Stromsburg and Osceola area and Monday through Friday in Shelby and Polk.

Home-delivered meals are offered for a suggested contribution. These meals are available to qualifying senior residents who reside in Polk County. Call Polk County Senior Services for more information.
Busy Wheels
This service operates in both Stromsburg and Osceola. To request a ride, call the senior centers. Stromsburg has ten volunteers that drive half a day each week and three relief drivers available on Wednesday, Thursday and Friday only. Osceola has three drivers and is looking for more. This may be just the volunteer position for you!

Programs Available for Seniors
Foot Clinics
Registered nurses provide basic toenail care and maintenance, as well as education to clients. Clinics take place in Crete, Dorchester, Friend and Wilber for a requested contribution of $20. Some towns are currently wait-listed. Please call for details.
Health and Fitness
Saline County Aging Services offers programs and classes to keep you healthy and active. Register for a Stepping On or Tai Chi class to improve balance, strength and to prevent falls.
Homestead Exemption Assistance
Homeowners are offered assistance in filing for Nebraska's Homestead Exemption. This tax break is available to homeowners age 65 and over and to persons with disabilities that meet specific income and valuation guidelines. To find out if you qualify, call Saline County Aging Services or the Saline County Assessor.
Meat Packages
All seniors age 60 and over in Saline County are eligible for a monthly meat package. Selection varies each month, but usually contains a mixture of chicken and pork products.
Medicaid Enrollment Assistance
We assist clients in filing for Medicaid benefits, including the SNAP program (formerly food stamps) and heating assistance. From application completion to faxing documents to assisting with telephone interviews, we are here to support you!
Medical Equipment Loans
Our office has a variety of well-maintained durable medical equipment. We offer short-term equipment loans at no cost to you.
Medicare Counseling
Staff members are trained to help you navigate Medicare programs. We assist clients in all aspects of the program including enrollment for new beneficiaries, understanding benefits, appealing decisions, comparing and signing up for a Part D program, comparing supplements and navigating all other aspects of Medicare paperwork.

Senior Meals
Nutritious noon meals served in a group setting are available in three locations in York County:
- York Senior Center serves meals Monday through Friday. Hot meals are served every Wednesday and Friday and on the first Monday of the month. An evening hot meal is served in lieu of a noon meal on the 3rd Monday. For all hot meals, reservations may be made by calling 402-362-7626 by noon one working day prior to the meal. A soup and salad bar is offered on Tuesdays and Thursdays from 11 a.m. to 1 p.m. and on the 2nd and 4th Mondays. Reservations are not necessary for the soup and salad bar. Pot lucks are held on the 2nd and 4th Tuesday and a fundraiser is generally held on the 3rd Thursday.
- York Leisure Home serves a hot meal on Tuesdays and Thursdays. Reservations need to be made by noon the day prior to the meal by calling 402-362-5900.
- McCool Junction serves a hot "homemade style" meal on Wednesdays and Fridays. For both meals, reservations need to be made by 8 a.m. Tuesday morning by calling 402-724-2542.
Meals are offered for a suggested contribution for eligible diners at all York sites. Eligible diners are those who are age 60 and over, the spouse of a diner that is age 60 years and over, a dependent adult child living with a diner age 60 and over, and disabled diners who live in an apartment where there is a senior nutrition program meal site (such as the Leisure Home in York). Non-eligible diners are welcome to join us for a meal, however they will pay full price for the meal (contact site for price).

Medicare and Insurance Information
Individuals with questions about Medicare, Medicaid and supplemental insurance can receive help from trained volunteers through the Senior Health Insurance Information Program (SHIIP). For an appointment with a SHIIP counselor, please call us at 402-362-7626.
Care Management Services
This program's care manager is trained to help older adults through difficult situations by providing a professional assessment of needs. Through available resources, those involved can make wise decisions. Following the assessment, the care manager will:
- Explain available services and all possible options.
- Make arrangements and help plan how to make best use of community resources.
- Help make arrangements to get services started.
- Continue to work with the client to make sure the plan is working.
- The fee for Care Management is based on the client's income.
In-Home Services
This program will assist arranging in-home services for persons age 60 and over who may need help with cleaning, cooking, laundry, shopping, errands, yard work, minor home repairs, snow removal, personal care tasks and respite care.
Match-ups are made with independent contractors in the community, or referrals made to appropriate agencies that provide needed services. The costs for these services are paid by the client. Low income persons may qualify for assistance in paying for needed services.
Transportation Services
These services are provided by York County Public Transportation:
- In-City limits: $2 or one punch per boarding – service in and around the city of York will be available from 7:30 a.m. to 5:30 p.m. Monday through Friday and from 9 a.m. to 1 p.m. Saturday. To increase efficiency, trips to and from some parts of the city may be limited to one time per day or certain times of the day.
- In-County: $4 or two punches per boarding – service to and from rural areas of York County to the City of York or to other rural areas within York County will be provided with 24 hours' notice (subject to driver availability).
- Out-of-County: $6 or three punches per boarding – out-of-county service is provided on Thursdays. Contact the office for current scheduled trips. Trips may be cancelled if there is not enough interest on a specific day.
- Vehicles equipped with a wheelchair lift are available with advance notice.
All stops must be pre-scheduled through York County Aging Services. There is a $2 charge or one punch per additional stop.
Passengers who find the cost of a full fare a barrier to using the system may apply for reduced rate tickets. Cost of reduced rate tickets are based on an individual's income compared to current poverty rates. To apply for reduced rate tickets, passengers will need to contact the York County Aging Services office and complete a fee determination form.
The following holidays will be observed with limited service provided: Martin Luther King Day, Presidents Day, Arbor Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, and the day after Thanksgiving. No service will be provided on New Year's Day, Thanksgiving Day or Christmas Day.
