Fees
  • $13.50 for a single vaccine (administration fee)
  • $10.50 for each additional vaccine (administration fee)
  • $5.00 for each copy of a vaccine record
  • $20.00 for an adult flu shot
  • $20.00 for an insured child's flu shot

Requirements for Childhood Vaccines

  • Children through 18 years of age
  • Must be accompanied by a parent or legal guardian
  • Bring records of previous vaccines
  • Vaccines are given to children at approximately; 2, 4, 6, 12-18 months of age, after the 4th birthday and in 7th grade (11-15 years of age).

Requirements for Flu Vaccines

  • Children 6 months through 18 years of age
  • Low income, uninsured adults - no Medicaid
