Personal and Family Services (PFS) Mission:

To help older adults avoid or resolve questions, concerns and difficulties that occur after age 60. Helping older adults remain independent in their homes is a continuing central purpose of the total program. Staff at Aging Partners Personal and Family Services, call on all the human service resources in the eight-county area of Butler, Fillmore, Lancaster, Polk, Saline, Saunders, Seward and York.

 Program Goals

  • To assure the safety, well-being and maximum independence of older individuals.
  • To increase the benefit older adults derive from human services by improving their access, understanding and appropriate use of those services.
  • To comprehensively address the needs of older individuals through inter-agency cooperation and the active involvement of friends and family.
  • To actively represent the needs of older adults and develop new answers to their concerns.

Aging and Disability Resource Center (ADRC)

The Aging and Disability Resource Center (ADRC) here is to assist Nebraskans in accessing services and support to help meet their long-term care needs.

ADRC will assist:

  • Persons age 60 and over.
  • Persons with disabilities of any age.
  • Family members, caregivers, advocates and providers for these groups.

What will the Nebraska ADRC provide?

Staff is available to help you with your questions and concerns about long term-care from 8 a.m. to 4:30 p.m. Monday through Friday. Call 402-441-7070.

We can help you with:

  • Living arrangements related to long-term care.
  • In-home services such as housekeeping, personal care, respite, equipment and care coordination.
  • Paying for long-term care services.
  • Health promotion and prevention.
  • Resources available to help you avoid and/or deal with abuse, neglect and financial exploitation.
  • Mental health services.
  • Financial guidance and resources, as well as other basic needs including food, shelter, Medicaid and Medicare.
  • Legal issues including consumer rights, advocacy, power of attorney and guardianship.

The ADRC is here to help you navigate through what can be a consuming and complicated maze of services and programs. We will collaborate and partner with multiple community providers to obtain and access information and services for you. All of our information and assistance is free and confidential.

Information and Assistance

Aging Partners' First Service desk is the starting point for obtaining information, determining alternatives and arranging services or referrals for those age 60 and over and their families in an eight-county area. The counselor's basic function is to connect people with resources in the community, provide information about a broad range of community services available,  and to share state or federal program resources they may not be aware of.

The First Service desk is primarily staffed by information specialists who can answer your questions regarding service delivery of your best care options. Call for help regarding:

  • Available senior programs
  • Caregiver issues
  • Home health care services
  • Medicare and Medicaid
  • Senior housing options
  • Transportation options

Our counselors are available weekdays from 9 a.m. to 4 p.m. except Wednesdays. In order to accommodate weekly staff training, service is available Wednesdays from 12:30 p.m. to 4 p.m. Walk-ins are welcomed, but appointments are preferred.

Care Management and Social Work

Care management and social work services provide professional assistance in assessing needs, planning and coordinating services for those age 60 and over. They help provide support to the more frail and vulnerable adults.

Our care managers are professional social workers and nurses trained to assess the living situation of frail or vulnerable older people and to find community services or resources to help meet their care needs. Our care managers arrange for a home visit to meet the client in their home environment; providing a better perspective in determining the service required. Care planning begins with a comprehensive assessment of the each person's needs, abilities, circumstances and support. From the assessment, a detailed plan for home care is developed. With the client's permission and participation, Aging Partners Personal and Family Services arrange and coordinate the delivery of various in-home services. Each client's needs and care plan are reviewed regularly. Adjustments are made, as necessary, to maintain his/her safety and quality of life at home on a long-term basis.

Social work service is a short-term relationship with the client. Our Social workers furnish the same professional services as case management, but it is less intense and more temporary in nature. Our social workers address difficult changes, re-certification or clarification of applications, and work with other agencies on behalf of the client.
