StarTran has updated its paratransit eligibility process. Effective June 1, 2026, applications can be submitted online, by mail, or by fax through FBA Nebraska.

Obtain Application

Two forms are required to register for the program: one to be completed by the applicant and one by the applicant's physician. You may obtain application forms in any of the following ways:

Download, complete, and print the application forms(PDF, 433KB).
Request that an application be mailed to you by FBA Nebraska.
Pick up an application at the StarTran Office, 710 “J” Street, Lincoln, NE 68508.

Submit Completed Application

The completed application should to be returned to FBA Nebraska. There are several methods of submitting the application:

For the fastest and easiest application process, individuals may apply online through the secure portal on the FBA Nebraska website. Please attach the completed forms and a photo for use on the ID card.
The individual can mail or fax the completed application to the address below. Please include a photo smaller than 1 inch by 1 inch to be used on the ID card. An ID card and program information will then be mailed to the individual.
